We have a shared vision with you for safe, sustainable and accessible health and care services provided as close to your home as possible. This is part of our strategy for 'A Healthier Mid and West Wales'. We are planning for how we best provide primary care and community-based services in Carmarthenshire, Ceredigion and Pembrokeshire.

These are most of the NHS-provided health and care services in your area, apart from hospital-based services. We want to talk with you about how that could be done in your local area.

You will sometimes hear your area described as a ‘cluster’. This describes how lots of local services involved in health and care in a geographical area work together. The aim is to better co-ordinate the well-being of people and communities.

There are seven clusters in the Hywel Dda University Health Board area. We want to share with you how you can access services from Primary Care and Community teams across all Hywel Dda clusters.

We want to know how you use health and care services now, and what you need to take care of yourself. Tell us what services you use, how you look after yourself and what matters most to you. We want to consider the opportunities and ideas you have. Whilst we must work within the finances we have, could we do things differently? We are inviting you to consider a day in the future, five to ten years from now. What would you like your health and care to look like? What would help you most in your community?
