What is available to help me with my health and well-being in North Pembrokeshire?  

The North Pembrokeshire area is bordered by Carmarthenshire to the east, Ceredigion to the northeast, and the coast. Haverfordwest is the largest town in the area and the county is home to the Pembrokeshire Coast National Park.

There are lots of services based in the North Pembrokeshire Cluster that can support you with your health and well-being.  

What is a Cluster? 

You will sometimes hear your area described as a ‘Cluster’. This describes how lots of local services involved in health and care in a geographical area work together. The aim is to better co-ordinate the well-being of people and communities.

The North Pembrokeshire Cluster has a population of 66,854 people and around 29,700 households.

The North Pembrokeshire Cluster has:  

  • 17 community pharmacies
  • 8 optometric practices (opticians)
  • 11 dentists
  • 8 GP practices;
  • 1 hospital (Withybush Hospital)
  • 35 nursing and residential homes
  • 4 integrated community networks

There is a range of other community services that can provide care and support in, or near to, your own home.  

The aim is to provide most of your care as close to home as possible. This can keep you well for longer and avoid you having to travel to hospital when you don’t need to. These teams help to get you home from hospital more quickly when you are ready.  

We also want you to be supported to make choices about your own health and well-being, and be empowered and supported to live the best quality life.  

About our population  

Much of our area is rural, with urban pockets, and we have more older people living here than other parts of the country.  

People are living with multiple health problems. This means they can need health and care for lots of different reasons, and from different professionals at the same time.  

In North Pembrokeshire locality we know that the greatest health risks/conditions are:

  • hypertension
  • diabetes
  • asthma

Below are some examples of cluster-funded projects that aim to support the health and well-being of this local population. 

  • The First Contact physiotherapy service in GP Practices to enable patients to have timely and appropriate access to Musculoskeletal (MSK) services.
  • Diabetic Foot Health Project to improve diabetes care in housebound and cohorts experiencing homelessness or housing insecurity. The team will also seek out non-attenders for diabetes review in the north cluster and proactively engage
  • Care co-ordinator multidisciplinary team, these teams will proactively support the early identification of people’s needs through multi-professional and organisation care planning and the provision of timely and accessible support.
