What is available to help me with my health and well-being in Tywi Taf?

The Tywi/Taf area describes much of west and north Carmarthenshire, including the county town of Carmarthen in the centre, as far west as Whitland, beyond Llandovery in the north, the southern coast of the county, as far east as Kidwelly.

There are lots of services based in the Tywi/Taf that can support you with your health and well-being.

What is a Cluster?

You will sometimes hear your area described as a ‘Cluster’. This describes how lots of local services involved in Health and Care in a geographical area work together. The aim is to better co-ordinate the well-being of people and communities.

Tywi/Taf Cluster has a population of 62,100 people and around 26,900 households.

The Tywi/Taf Cluster has:

  • 13 community pharmacies
  • 11 optometric practices (opticians)
  • 7 dental practices and 1 orthodontic practice
  • 8 GP practices;
  • 1 hospital (Glangwili Hospital)
  • 1 community hospital (Llandovery Hospital)
  • 11 nursing and residential homes
  • 1 community resource team

There is a range of other community services that can provide care and support in, or near to, your own home.

The aim is to provide most of your care as close to home as possible. This can keep you well for longer and avoid you having to travel to hospital when you don’t need to. These teams help to get you home from hospital more quickly, when you are ready.

We also want you to be supported to make choices about your own health and well-being and be empowered and supported to live the best quality life.

About our population

Much of our area is rural, with urban pockets, and we have more older people living here than other parts of the country.

People are living with multiple health problems. This means they can need health and care for lots of different reasons, and from different professionals at the same time.

In the Tywi/Taf Cluster we know that the greatest health risks/conditions are:

  • hypertension
  • diabetes
  • frailty
  • mental health

Below are some examples of cluster funded projects that aim to support the health and well-being of this local population.

  • Support with frailty and how to reduce emergency admissions. We aim to support our local population to remain in their own home; with an emphasis on population wellbeing and community connection by establishing greater links with partner services. The Cluster is now currently recruiting for a carers assessment co-ordinator to support this.
  • Supporting people at a greater risk of needing urgent or emergency care by aiming for integration and a greater emphasis on anticipatory and preventative care. The Cluster meets monthly with all professional leads.
  • Providing mental health services like MIND for low to medium mental health symptoms and PAPRYUS suicide prevention support for young people.
