What is available to help me with my health and well-being in South Pembrokeshire?
The South Pembrokeshire area is bordered by Carmarthenshire to the west and Ceredigion to the south and the coast. The county is home to the Pembrokeshire Coast National Park and a busy tourist destination where the population of some towns can double in size during the holiday periods.
There are a range of services in South Pembrokeshire that can support your health and well-being.
What is a Cluster?
You will sometimes hear your area described as a ‘Cluster’. This describes how lots of local services involved in health and care in a geographical area work together. The aim is to better co-ordinate the well-being of people and communities.
South Pembrokeshire has a population of 54,528 people across 24,100 households.
South Pembrokeshire Cluster has: 
- 13 community pharmacies
- 5 optometric practices (opticians)
- 4 dentists and 1 orthodontic practice
- 5 GP practices with 3 branch surgeries
- 2 community hospitals (Tenby Cottage Hospital and South Pembrokeshire Hospital)
- 21 nursing and residential homes
- 4 integrated community networks
- 1 Pembrokeshire community hub
- 1 walk-in centre (Tenby Cottage Hospital)

There is a range of community services which provide care and support in, or near, your own home.
Our aim is to provide most care as close to home as possible. This can keep you well for longer and prevents travel to hospital, unless necessary. These teams help to get you home from hospital quickly, when you are ready.
We want to support you to make choices about your own health and well-being and be empowered to live the best quality life.
About our population
Just over 99% of the population in this cluster, live in a rural area. The area is a busy tourist destination, and the population can almost double in size during the summer months.
People are living with multiple health problems, and this means they can need health and care for lots of different reasons, and from different professionals at the same time.
In South Pembrokeshire Cluster we know that the greatest health risks/conditions are:
- respiratory disease
- heart disease
- frailty
- mental health
Below are some examples of cluster funded projects that aim to support the health and well-being of the local population.
- Improving children health – We have two Cluster projects in place. One is improving asthma in primary schools. The other is our young mental health project for children aged 5 to 18 years of age, offering low level mental health and emotional well-being support.
- Integrated Community Network (ICN) – Our multi-professionals work across different services and together. The ICN targets support for our older population and people with long-term conditions, in particular. This is helping to avoid unnecessary admissions and supporting patients to stay in their communities
- Each of the GP practices in this area has a first-point contact service for physiotherapy so people can get the help they need quickly.
- We are looking to provide local and easily accessed spirometry testing. This measures how much air you breathe in one forced breath to help diagnose and monitor certain lung conditions. The aim is to achieve no waiting list for future testing.
