North Ceredigion Cluster
Consultation has concluded
What is available to help me with my health and well-being in North Ceredigion?
The North Ceredigion Cluster describes from Aberaeron and Tregaron, up towards the south Gwynedd and Powys borders, including Aberystwyth and Borth communities.
There are lots of services based in the North Ceredigion Cluster that can support you with your health and well-being.
What is a Cluster?
You will sometimes hear your area described as a ‘Cluster’. This describes how lots of local services involved in health and care in a geographical area work together. The aim is to better co-ordinate the well-being of people and communities.
The North Ceredigion Cluster has a population of around 41,100 people and around 17,000 households
The North Ceredigion Cluster has:
- 9 community pharmacies
- 5 optometric practices (opticians)
- 4 dental practices
- 7 GP practices
- 1 hospital (Bronglais Hospital)
- 1 integrated care centre (Aberaeron)
- 13 nursing and residential homes
- 1 community resource team (based in Aberaeron Integrated Care Centre)
You can find out how some of these services can help you, and which is best to help you in different circumstances, by visiting our web resource and My Health My Choice video series here (opens in new tab).
There is a range of other Community Services that can provide care and support in, or near to, your own home.
The aim is to provide most of your care as close to home as possible. This can keep you well for longer and avoid you having to travel to hospital when you don’t need to. These teams help to get you home from hospital more quickly, when you are ready.
We also want you to be supported to make choices about your own health and well-being and be empowered and supported to live the best quality life.
About our population
Much of our area is rural, with urban pockets, and we have more older people living here than other parts of the country.
People are living with multiple health problems. This means they can need health and care for lots of different reasons, and from different professionals at the same time.
In North Ceredigion, we know that the greatest health risks/conditions are:
- Heart Failure
- Cancer
- Rheumatoid Arthritis
- Mental Health Conditions
Below are some examples of cluster funded projects that aim to support the health and well-being of this local population.
- Women’s Health Psychology Service – this is being pioneered to help alleviate some of the wellbeing and mental health issues that are often associated with conditions such as Endometriosis, Pelvic Inflammatory Disease, and the Menopause.
- Area 43 – this is a counselling service, that typically offers six counselling sessions to patients aged 13 – 30.
- Early Intervention Pain Service – this Service caters to people living with chronic pain. It has worked extremely well to minimise waiting lists in hospitals. It provides patients with timely psychological support while they are living with chronic physical conditions.
