Meddygfa Penrhyn - Patient Questionnaire

Meddygfa Penrhyn was formed in November 2024 when patients and staff from St Davids Surgery transferred to the Health Board managed practice at Solva Surgery.  At the same time a small branch surgery was established at Shalom House in St Davids.

We are now several months into the changes and the Health Board would like to hear from all patients across the Peninsula to learn what you think about the service at Meddygfa Penrhyn and how the changes may have affected you.  Please take a few moments to complete this anonymous questionnaire.

We would like to establish a Patient Participation Group to bring together patients, carers and practice staff to improve the experience from the patient perspective.   This Group would meet quarterly and is looking for volunteers – would you be interested in helping?  If so, please give your contact details below.
